Required Paperwork

Along with completing and signing the necessary enrollment forms, a copy of a child registration card and current immunization records are required prior to completing the registration process and officially enrolling a child.

A pre-admission statement of health form completed by a pediatrician, family physician, or registered nurse must be provided for each child prior to attendance and also on an annual basis. The policy held by most medical offices is that a health statement can be authorized after a child has had a well-check visit or physical examination performed.
